.....you need to know whom you can count on for support and care. Welcome to Hospice & Palliative Care of Cape Cod. We’re here for you, wherever you are, whenever you need us most.

Founded in 1981, Hospice & Palliative Care of Cape Cod is the area’s first and only independent non-profit organization offering hospice services. We are the hospice provider most often cited by local health care professionals as having the best reputation, the one they say their patients would most prefer to use, and to whom they would most likely refer their own family members. End-of-life care is what we do. Our commitment to this important cause means that you and your family receive the benefits of our long-term expertise and dedication, with an emphasis on comfort and caring, rather than curing. We create customized personal support and treatment plans that meet the physical, emotional and spiritual needs of both patient and family, carried out by our interdisciplinary team.

H&PCCC recently created a collaboration with Hospice of Martha’s Vineyard and the Vineyard Nursing Association to form Island Hospice, the first Medicare-reimbursable hospice for residents of Dukes County. We also purchased Hospice of Boston & Greater Brockton, a small non-profit with a 30 year legacy of excellent care.The sum of the three programs provides one contiguous service area ranging from Provincetown to Boston, including all of Barnstable, Dukes, Plymouth, Bristol, Suffolk and Norfolk Counties.

Our staff of over 175 medical and support professionals and more than 200 trained volunteers serve over 1,000 patients a year, most in private homes, as well as in long-term care facilities, hospitals and our own gracious McCarthy Care Center. We also offer grief support services to those experiencing grief or loss, including children through our unique Kids Grieve Too! Program. Community education is available on a wide variety of topics through our Speakers Bureau. We welcome volunteers in many different roles and provide in-depth screening, training and ongoing support. Donations to help us insure that these services can continue to be offered to all regardless of ability to pay. Anyone may make a referral any time 24/7 at 508-957-0200 or 800-642-2423.

Hospice of Boston & Greater Brockton seeks Volunteers for Spring Training

Mike Chorzewski has been a volunteer for the Hospice of Boston & Greater Brockton (HOB) for the past two years. He is often asked “How can you do it?” He finds the work so rewarding that he responds, “How can I not do it?” Chorzewski is grateful for his role in having helped patients and families cope with the impact of serious illness over the years. He says, “Hospice has helped my family. It meant the world to us. Volunteering is my way of doing something special for someone else.”

Hospice to provide primary care at home

The day of the house call is back for some Cape Cod patients with chronic illness.

An official with Hospice & Palliative Care of Cape Cod said Monday that the organization will expand its services to include home-based primary care for the housebound and a home-based support program for people who care for dementia patients.

Remember when doctors made house calls? It looks like we're coming full circle, at least for patients with chronic conditions.

Hospice & Palliative Care of Cape Cod recently announced that it will expand its services to include home-based primary care for the housebound and a home-based support program for people who care for dementia patients.

The aim of the new services is to provide care in the home so patients can spend less time in costly recurring visits to the hospital.

Five Wishes is the first living will that talks about personal, emotional and spiritual needs as well as your medical wishes. It lets you choose the person you want to make health care decision for you if you are not able to make them for yourself. Five Wishes lets you say exactly how you wish to be treated if you get seriously ill. It was written with the help of The American Bar Association's Commission on the Legal Problems of the Elderly and the nation's leading experts in end-of-life care. It's also easy to use. All you have to do is check a box, circle a direction, or write a few sentences.
