Under the supervision of the Director of the Psychosocial Dept., is responsible for planning, implementing, and coordinating bereavement support groups and impending loss groups for individuals requesting services. Facilitates bereavement support groups as needed. Is responsible for assessing the suitability of individuals for group support or individual/family counseling. Provides grief education to staff and community member as needed. |
- Currently licensed as LICSW in MA
- Prior clinical experience relevant to health care, medical social work, or oncology required
- Knowledge of community resources

Intake Coordinator
Full-time, weekdays

Under the direction of the Referral Center Director, is responsible for receiving and processing all telephone inquiries and referrals in a timely fashion. Provides brief description of hospice services and philosophy to potential patients and families. Assists in initial assessment of eligibility for incoming referrals. Verifies reimbursement sources. Notifies billing dept. of new patients. Reviews all admission documentation. |
- Current license as RN /LPN in MA
- Knowledge of hospice philosophy of care and hospice enefit preferred
- Experience in hospice, home health, or hospital intake and marketing preferred
- Excellent assessment skills

Child & Adolescent Counselor
Full-time
|
Under the supervision of the Director of the Psychosocial Dept., is responsible for the provision of expressive therapy services to children,adolescents, and their families. Assesses the developmental, educational, emotional, social, and spiritual needs of children and adolescents experiencing loss due to illness or death. Provides grief education to the community regarding child/adolescent and grief issues, therapeutic support and intervention strategies. |
- Currently licensed as LMHC in MA, preferrable LIMHC
- Masters degree in art therapy, clinical social work or counseling psychology
- Prior experience working with children/adolescents in individual and group settings; proficiency in providing expressive therapy
- Cumputer literate

CNA
24 hours/week 7am to 7 pm - rotating 5 week schedule

Working at the 10-bed McCarthy Care Center, under the direction of an RN, provides care and support to patients. Provides personal care, housekeeping and nourishment for patients. Recognizes and reports changes in patient's status. Provides support to patient and family through on-going communication. Documents care provided per agency policy. |
- Current CNA certification
- HHA certification preferred
- Previous hospice experience preferred
- CPR certified within 30 days of hire

24/hours/week 7pm to 7am - every Sat & Sun

Working at the 10-bed McCarthy Care Center, under the direction of an RN, performs selected nursing care to patients. Assists the RN with preparation, implemen-tation, documentation, and continuing evaluation of the skilled nursing plan of care. Documents in electronic medical record. Provides support to patient and family through on-going communication.

Per Diem Home Health Aide
|
Covers vacations and increased patient census in the field. Performs personal care and light house-keeping in accordance with the HHA assignment sheet. Recognizes and reports changes in patient's condition to appropriate staff. Provides support to patient and family through on-going communication. Documents care provided per agency policy. 12 hours of annual education required.

- Current CNA/HHA certification
- Prior homecare experience preferred
- Previous hospice experience preferred

Conducts assessments and admissions for home, nursing home, and hospital visits, describing and explaining hospice and home care services to potential patients / families. Describes hospice philosophy and services to potential patients / families. Informs patient / family regarding financial obligations and insurance coverage using knowlege of fees, costs and reimbursement source. Explains Medicare benefits to patient / family, presents other third party insurance options. Using laptop computer, enters data into electronic medical record. |

Working in the After-Hours program, responds to calls from Triage RN to deliver service to current patients. Assesses patient's status based on the physiological, psychosocial, spiritual, and emotional needs presented. Implements the plan of care under the direction of the attending physician. Using laptop computer, enters data into electronic medical record. |
